I can't access my form.

It appears that your account has been autosuspended by our Phishing detector. That is why your forms have been disabled. Please note that collecting login credentials is a violation of jotform's terms.
Here's an excerpt from our terms:
"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
To remove the suspension on your account, I would suggest you delete the suspected fields.
Once you have deleted the said fields, let us know and we will lift the suspension accordingly.
