Your forms are disabled because your account has been autosuspended by our phishing detector. However, I checked all your forms and they don't seem to be violating jotform's terms so I have now lifted the suspension on your account.
You should now be able to view your forms without any issue.
In the future, I would suggest not using "Sign-in" in your title as our phishing detector might flag that as a suspicious form.
