I have checked you account and the following are the details; it is ACTIVE and on PREMIUM package.

You currently are on a active premium account. You can view this by going to 'Account Settings' can be found on the bottom right of the Jotform homepage.

Here's an easy way to see your account details.
1. Login to your JotForm account
2. At the bottom of the page (right side), you will then see Account Setting
3. Click Account Setting button
