I keep getting the "Form is disabled" error on a survey

I keep getting the error that my form is not found when I attempt to preview my survey: https://form.jotform.com/61125117188149.

I am attempting to create a test survey, but it keeps telling me it is disabled. I reviewed the Terms of Service, but do not see any particular violations that would prevent this from being used as I do not collect any personal information on it. Please advise.
