We have used 6 of 20 submissions? But I am receiving emails that our account is disabled.

I checked your account and was under overlimit, but i have activated it, it should it be ok now!. Note that you have submitted 6 forms and 5 are active so meaning you are getting overlimit as it is set at 10.
In case you get the submission overlimt, you can register new free account using same email account to enjoy more submission offers. Be also thinking of upgrading your account to PREMIUM to enjoy more submission offers

As far as i can see, you are using a secure version of your form. May i suggest you to use a regular form? You just need to change your forms URL from https://secure.jotform to http://form.jotform or http://www.jotform.
You can also create a free account to increase your monthly submissions limit. Guest accounts are limited to receive 20 regular submissions, 2 SSL submissions and 2 Payments(whichever comes first) per month. While you are creating your account, please enter the same email address where you receive your form notifications. So your form will be automatically transferred to your new, free account.

I made an account but now my form is gone and the past submissions. Can you please send the past submissions to my email email@example.com?

Hi, you can forward past submission by yourself.
Login with old account,select form, click Submission and you can manage them.

Please confirm that you can access it.
Please note the form is still using a secure URL on the website address given above, and 'Free' accounts only get 10 SSL submissions per month.. if you think you will have more submissions than that, you should consider using using a non-secure URL in your embed code.
