    Unfortunately, we do not have any database program or widgets that you can use in your form. But you can send your form to your MySQL database if you want. Here's an instruction http://www.jotform.com/help/126-How-to-Insert-Update-Submissions-to-Your-MySQL-Database-Using-PHP.
