Why is my form disabled?

    I can see that the form you are pertaining to is with a guest account. I have checked the form and I saw that it has a field that caused your form to be disabled.

    Please be informed that Jotform does not allow fields to be used to collect login information such as above. Jotform has an anti-phishing system that scans forms 24/7 and marks it if suspected of any phishing activity. Phishing as stated in your Terms of Use is strictly prohibited.

    I would suggest you try to sign-up to an account with the same email address you've used in creating the form or opening this thread. Once you've done that, we can delete the fields that caused your form to be disabled and activate the account.
