I can see that the form you are pertaining to is with a guest account. I have checked the form and I saw that it has a field that caused your form to be disabled.
I would suggest you try to sign-up to an account with the same email address you've used in creating the form or opening this thread. Once you've done that, we can delete the fields that caused your form to be disabled and activate the account.
