    Kindly which code app are you using to read your jotform codes? Is it a custom one or 3rd party? This could be an app issue.

    Please check this step by step instruction on how to reinstall the form on your Facebook page and let me know if it works. You may be using old method to embed forms.

    This issue has already been reported to our developers it appears that the app integration method on facebook does not work for others.

    Please give it a try using the static HTML method as suggested by Matthew.

    Once the Static HTML method has been used once, how do you go back and add other surveys using the same method?

    I am having the same issue as LuxeForLess and it started when we switched to the new FB timeline.  My Static HTML app is still working however my other 2 surveys (which were installed w/the developers method) are no longer working
