I am trying to logon to my account to make changes and cannot recall the email and password used??

We understand that you are having trouble accessing your form account due to not being able to recall the email and password used originally.
I have attempted to look up your information using your email address above "email@example.com" and I found you have a jotform username for "martyb" which currently has two forms on it.
Please try using the above and reset your password as well and you should receive an email to the above email address that is associated with this account to access to begin using it again.
If you don't receive it then please check your spam folder as it may have went there.
Otherwise if this is still an issue or it is another account you are trying to access let us know.
