Can you please try using iFrame method to embed your form?

To get your form's iframe codes:

    1.  Click Setup & Embed tab
    2.  Click Embed Form button
    3.  Choose iFrame option

    4.  Copy the codes provided in the succeeding screen

If it does not help, please provide the URL of the exact page where the form is embedded so we can inspect it and come up with an appropriate suggestion.

We look forward to hearing from you. Thanks!
