How to hide/show fields base on users answer

Is there a way to ask at the beginning of the form, "Do you already have an appointment scheduled?" And if they say yes, they can continue filling out the form. If no, the rest of the questions are not seen? I'd like for it to pop up with our phone number if they said they don't have an appointment.
