Why is my form disabled?

When I try to view my form at https://form.jotform.com/61274348374157 it says it is disabled, but I have explanation as to why this is.  Please help!

Hi, sorry I wasn't sure where to go to see the reply to this question and so I didn't delete the fields in time.  They have since been removed.  Can you please enable my form?
