when i test my form, i cannot send it. its always disabled. i dont know why. i can make the form, change it, visit it and ask to his questions, but when i send it, i cannot. it always said that my form was disabled.

Several of your GUEST accounts have been AUTOSUSPENDED for several times already due to Phishing contents of the Form. May I suggest you register officially and create a FREE account.

In addition to jcm0731 answer, I found out that there's no Form inside your account.
