Is Jotform HIPAA compliant? And if so, do you provide a BAA?

I am a hypnotherapist and I want to use Jotform for my new client intake form.  My website has a client dashboard that is password protected. I want to embed the form in a HIPAA compliant email to new clients sent from this dashboard.  I have several questions about how Jotform works

1- Is Jotform HIPAA compliant? And if so, do you provide a BAA?

JotForm complies with technical safeguards but we don't have certificate for either HIPPA or BAA, please refer to this link for complete information regarding the HIPPA:

Great news! JotForm now offers HIPAA compliance. This means users in the healthcare industry can use JotForm to collect sensitive patient information through consent and onboarding forms, medical history updates, online bill payments, and prescription refill requests. 

HIPAA-compliant forms require a Gold pricing plan, which is only $99 a month or a Silver pricing plan, which is $39 a month. A business associate agreement (BAA) is also available upon request.
