Jot Form Logo

After I have made the form a shortcut on the iPad screen how can I change to Jotform logo to my own unique image?

If I understand correctly, you are using the Add To Home Screen widget to add the form in your shortcuts and you want to change the logo with your own image? To do so, simply click on the wand icon on the top right of the Add To Home Screen widget.

Then insert the url of your logo in the Logo Image URL field.
