I presume you are referring to the fillable PDF form? Please check this guide about it: https://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator. There are times that the exact design of the form cannot be translated in the PDF version.
This is how it looks like when I cloned your form and converted it to a fillable PDF:
As you have mentioned, I presume the problem is the size of the labels of your input fields? Currently, we cannot edit them, I believe the system generates a fixed size. The ones that you can edit is the headers and static text fields, you can format them in the form builder and they will show up the same way on the fillable PDF.

Unfortunately, if you are replying in this thread using your email client and attaching files, it won't go through here. You need to upload your PDF file in a storage service like Dropbox or Google Drive, then directly send us the download link to it. I presume the screencast I have showed is the same problem you are getting?
