Could you share us the form that you are adding in the templates gallery?
May we also now if you are unsharing it or deleting the actual form that was shared on the template gallery? After you unshare it, you should be able to see it like this when you open the share interface:

I searched for your form by checking your username and was able to see that the form is still in the Templates Gallery.

However, please do let us know if that template suddenly disappears, including its direct link. We actually have a lot of form templates and it might be quite hard to look for the specific template if you'll do a manual search of it.

- eramirez0730Answered on May 16, 2016 at 02:45 PMI named it “5885” so that it would be easy to search. My peers are usually able to find it for a couple of days by searching “5885” but after that, it does not come up on search anymore.

That is quite odd. I have tried and search for the word "5885", and I was able to see the form template immediately. Here's a screencast of it:
Could you let us know if after a day or two the form template cannot be found again? It might be something related to the tags on your template or the search function in the templates gallery's database. For now, please do monitor and let us know immediately if the problem arises again, we'll investigate it immediately then. Apologies for the inconvenience.
