PDF report: Header image is missing from heading field

Hello,

On a couple different occasions now, I have tried inserting our logo image into the pdf output and it shows up in the pdf editor. However, when you actually get the pdf output, the image isn't included Is there a way to embed an image to pdf.

Any advice?

I was able to replicate mentioned issue when I created PDF report of your form submission. I was also able to replicate mentioned issue on my cloned form and on my sample form. 

I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved. 

For now as possible workaround you can add logo image to your form using image field instead of adding image to Heading field.
