I was able to see the problem as well when I test the form on your website using mobile browser.
What I suggest you try first as resoluiton is to re-embed your form https://www.jotform.us/form/43315343161142 on the website using its iframe embed publish code instead.
Make sure to clear/remove first the previous script embedded code of the form before adding back using its iframe code.
The issue must be happening due to script conflict with the other jquery scripts existing on your website and the form scripts. Using the iframe embed method should prevent the script conflict.
Let us know if this still did not fix the problem.
Hi Jonathan, sorry my response is soooo delayed. I embedded the iframe code as instructed, but the result is the same (not viewable on mobile platform). I've made the change for http://www.pedaltherapy.com/forms/registration/, (embedded iframe) but have left http://www.pedaltherapy.com/forms/feedback/ unchanged (no iFrame data) for comparison.
It seems that your form is embedded under a div with a mediumScreenDisabled class. That class is hiding the div on small screens.
I would like to suggest checking this issue with your website builder/theme developer.
