If your form is embedded to a high traffic page, each time the page is loaded, the form will be loaded also. Each time the form is loaded, it counts towards your form views.

If you want unlimited form views limit, you can allow JotForm to display ad (Powered By JotForm) in the footer section of your forms. To do so, just check "JotForm Footer" on your settings page https://www.jotform.com/myaccount/settings. You should see "unlimited used" in your account status after that under Form Views.

I reactivated the account, everything should be back up now.

Keep in mind, you will need to activate the footer on the account that shared the forms with you. In this case, from the account "MonicaTew". Activating the footer on your account will not activate it on the forms that are shared with you by another account.

Just out of curiosity, Monica Tew is no longer affiliated with the company.
Is there any way this form (and the others shared to me) can be transferred
to a new account?

We can only move forms at the request of the account that holds them. If you can have her request they be moved from her account to yours, we will be happy to move them.

I was able to get her login info and made the necessary change. She has
purged all "non" company related forms. Would it easier for me to change
the account name, password and email settings versus moving everything
(since the links are already established/published to the web)?

The links and such will not change if we move the forms. Form ID's remain the same when we move forms so all embedded forms will not be affected. If you are OK with using her account and updating the everything, that can be done or we can also move the forms to your current account. Either way, forms would be not be affected.
