I work at The Gallery at Military Circle Mall in Norfolk, VA, and I am looking into thrid party websites. My marketing coordinator, Melissa Smith, and I are responsible for driving more traffic to our FB page and gaining more likes. We have a lot of contests and giveaways, and wanted to know was there a way to have the form directly put on FB for our fans to fill out. Also, is there a way to track the names of people that like our FB page? We were also wondering if it would possible for the mall/company to receive an invoice instead of paying by credit card. I look forward to hearing from you.

You can actually track your page statistics through your Facebook Fanpage's Admin Panel and "Insights". Form more information on how to use this, you can check this: http://www.facebook.com/help/pages
In relevance to your question about Jotform, you can actually integrate paypal payments on your form. With this, after paying on your page, your subscribers will receive an invoice on their email from Paypal.
In order to achieve this, first you have to set up a form, run the paypal wizard (here is a tutorial: http://www.jotform.com/help/50-PayPal-Forms) and then embed the form on your Facebook page using this: http://www.jotform.com/help/125-Adding-forms-to-Facebook-Static-HTML-App-Method-
