The fields are being cleared when you submit the form due to the settings of "Clear Hidden Field Values" in the Preferences. Currently it is set to Clear on Submit. To resolve the issue, go to Preferences > Advanced Settings > Clear Hidden Field values and set it to Don't Clear.
