It appears that your account was autosuspended by our phishing detector. This is why you are getting an error when viewing the forms. Please note that collecting username and password is not allowed in jotform. Please see our terms for more info: http://www.jotform.com/terms/
If you want to remove the suspension on your form, I would suggest you delete the fields that are violating our terms.
Once you have done so, let us know on this thread so we may lift the suspension.
