I see that your account scsezgin is over limit and hence the forms should be displaying Overlimit error message on the form. I see that your account has already received 3600 form views. Please be noted that the Free accounts are limited to receive only 1000 form views/100 submissions/10 payments in a month. If you are expecting more submissions/views, it is required to upgrade you account to a paid plan. Or you may wait for the submissions/views counter to reset on the first day of the following month.

I see that the account is now holding Bronze subscription plan. It seems that you want to upgrade the account for one month. I see that the account is scheduled to downgrade to Free by 20 Jun 2016.
