I was actually able to check your account and it seems that you have reached your monthly quota for uploads. This is why your form was disabled. Please upgrade your account or clear some space on your account to allow further submissions. Please refer to this page for pricing plans:

Upon checking you account, I found out that you are using a free account and you already reach the required submission limit for a free account. That's why your form is Disable. I suggest upgrade your account to premium were you can received 1,000 submissions per month, 1,000 SSL Secure submissions, you can received 1,000 payments submission per month. However, if you don't want to upgrade your account, you can wait until submission automaticaly resets to zero. The automation reset will be done first day next month.

    Thank you!

You can clear some space from your allocated space by deleting old submissions that contain uploads.

You are way over the limit of the upload space allocated to 'Free' accounts.
