I TRIED TO ATTACH A SCREENSHOT, BUT IT WAS TOO BIG.
PROBLEM IS, AFTER I "GRANT ACCESS" FROM BOX FOR THE INTEGRATION, I AM TAKEN BACK TO THE INTEGRATION PAGE WITHIN JOTFORM, BUT GET THE FOLLOWING MESSAGE:
'Authentication Failed! Please check your credentials.'
REEEEEEAAAAAALLLLLLLY FRUSTRATING AS I HAVE SPENT THE PAST 16 HOURS PUTTING MY FORM TOGETHER.
WHAT CREDENTIALS DO I NEED? I CURRENTLY AM USING A TRIAL MEMBERSHIP. DO I HAVE TO PAY IN ORDER TO BE ABLE TO INTEGRATE??

There should not be any other credentials needed once you have logged in to your box account and integrated it with your form. Can you please try clearing your browser cache and cookies before reintegrating the form with Box again? Here's a guide that can help: How to clear your browser's cache.
