How can I change the format of the PDF?

I'm trying to put together a letter that would be sent out to our membership.  Upon submission, the PDF would be sent to the requester as a letter format with a signature line that would then be routed to the member.

Is there anyway I can create a letter format instead of a table formatted PDF that would include a signature line?

This does allow for slight customization so it still may not be fully within the scope of what you are looking for but it should allow for a more specific layout.
