I have checked your form and I can see you created it using a guest account, the limit for that accounts is very low and I would suggest you to create an account, otherwise your form will be disabled or when the cache or browser data is cleared the guest account will be cleared as well. This is why I cannot actually check which email address you used in your form.
We can also move the form to your already created account as well, only let us know the username used and we will transfer the form for you.
