The Phishing filter had auto suspended your account/username laks forms due to some of your forms that have the 'password' text on them.
But I checked manually and found the forms were not phishing forms. I have corrected the mistake and have now reactivated your account and forms. You may need to relogin to jotform at https://www.jotform.com/login to see the updated status.
