BJoanna was able to help me BUT I noticed that after I cloned and embedded the form on multiple pages on my website, the form was no longer responsive. Can I not have the same form on multiple pages? It seems to break the respsoniveness when added on multiple pages.

This should not be an issue, the form should be displayed the same does not matter the number of pages where you have embedded the form.
I checked the link you shared and I can see the form embedded there is working responsively.
May we get the all the links where you form is embedded?
I would also suggest you to embed your form using the same method in all the web pages.
We will wait for your response.
