You need to have the SQL information (authentication) ready in order to use webhook. You need to create the tables in your SQL server before using webhook.
We have a few apps that you can use to export your form submissions into various database format, please look at here: https://apps.jotform.com/search/export. You can give it a try and see if you can automatically create the tables on your SQL Server. Please choose the correct format.
I haven't use mostly of this app, but I can help you if needed.
