After the form is submitted, it displays QR code.

I did test your form by sending a test submission. After the form is submitted, it is displaying some QR code. Please check the screenshot below:

Is this the issue you are having? 

Please be noted that you seems to have added image in your form's thank you message and that is the reason why it shows up when the form is submitted. If you do not want to display it, you need to delete it form your form's thank you message. Please check the screenshot below:
