Missing Submissions - For Conference Registration

We have identified several submissions that have inexplicably disappeared from the form submissions.  The ones we have found so far, were submitted on May 17, 2016 (for example the full name of one of the missing submissions is Vince Otoupal).  We have the emails that show the submissions were made but these submissions cannot be found when we view the form submissions.  Please review and explain why these were removed. 

We've fixed the glitch that is causing this issue. Please check again and let us know if there are still missing submissions.
