Form no longer available?!

    Form is still listed in my forms, but links are broken!  Fix ASAP!

    I saw that you tried to create Custom URL of your form. URL that you provided is not correct and because of that 404 error message is shown.

    Marked part of your form URL is not correct. Instead of that part you need to add your custom name.

    You can edit or reset your custom name inside of your form builder.
