My first form got disabled

Hi,

I´m trying to make a form but it got disabled (form not found) just after I included a conditional "password".

Can you help me to get this form available again?

Your account was suspended automatically by our phishing tool. You are not allowed to collect passwords here in JotForm. This violates our terms of use. Actually, the word "password" in a form will trigger the phishing tool to suspend the user's account. 

I removed the password field in your form so that I can re-activate your account. Please try to log out and log back in to verify the changes.
