I see in your account that you have exceeded the allotted form views limits in your account. Please take note that free users are allowed 1000 monthly form views. You can learn more about form views from this article: https://www.jotform.com/blog/191-Announcing-Pricing-Modification-Form-View-Limits
But, there is a workaround in order to prevent incurring limits on form views. This is by enabling the JotForm footer on your form. You can do this by going to My Account > Settings (or click here) and tick the checkbox on JotForm footer.
For the meantime, I have restarted your form views counter.
