Problems with uploading document

I would like to upload a form from my documents and it wont let me.   it shows edit field and upload and it wont open my documents

Are you trying to upload some document to your form and you are unable to do it? Which type of document are you trying to upload? Maybe file extension that your are trying to upload is not allowed or maybe file is to large.
