How to create a comment box form

I want to make a form like the comment box that I am using right now to make my comment to you. I can see everyone else's comment and there is even a place for an answer.

We do not have a guestbook form type. However, this is possible to embed a report with the form answers to your web page or directly within the form page. Please see the next forum thread:
