How can I make my form totally responsive

I mind i need to see it in a smart phone complete, incluiding the background, or theme... not only the form.

I did mark the check box but it dont works, can tou help me please?

Have you tried with the Mobile Responsive widget? If that does not work for you kindly let us know.

Your form is already responsive. Can you please tell us if you are experiencing an issue? The form background is there. But the page background which is the image won't show up because of the screen size. 

Do you want to keep the alignment of the field label and input into one line? Let us know.
