Please note the following:

-Custom URLs names (ie: http://www.jotform.com/ruvosika/myform ) will not work since we have changed your account username. 

-However, direct form links like http://www.jotform.us/form/61676069413157 will be fine.

Your account username has been changed from "ruvosika" to "rustudentactivities". Open a new thread if you need anything else, we will be glad to assist you.
