How can I embed a form in my LionDesk email template for drip campaigning?

I have embedded the source code of the form in my email template successfully except when you receive an email (not on a mobile device) and open it via a desktop or laptop there are several bullets aligned next to each field name, the alignment is off center and clumped together somewhat and there is a hidden field at the bottom that appears title "This should be empty".  I did not add that field.

I attached a screenshot of part of the form.

Unfortunately, you cannot embed a form in the email template. This is because email clients are not able to read or execute Javascript and PHP code. For security purposes only HTML or another markup language can be translated or read.

If you want to include a form in the email template, you will need to copy the URL of the form and paste it on the email. Then the user will simply need to click on the URL to open the form on their web browser.
