We recently updated the Jotform and published it on our website.
Here's the issue:
A) If you go here: https://support.okta.com/help/articles/Knowledge_Article/Okta-Partner-Boot-Camp and then click on registration, and from the dropdown on the top of classes, you select Okta Partner Boot Camp, It will show you a list of dates of classes. All this information is correct.
B) If you go here, https://support.okta.com/help/training?_ga=1.47736740.2046583111.1461442625 , and then click on sign up link in the Okta Partner Boot Camp section towards bottom. It will take you to the same page as Part A. However, when you select from the dropdown of classes for Okta Partner Bootcamp, all the dates do not show. Only a few of them show.
Essentially part A and part B both share the same address:
A) https://form.jotform.com/OktaTraining/premium
B) https://form.jotform.com/OktaTraining/Premium
Is it because the letter 'p' in the word premium is lowercase for one and upper case for another?

After playing around with the your site a bit, I did noticed that when you open the site
https://form.jotform.com/OktaTraining/premium
The drop down field displays all the check boxes options.
But when you open the other URL
https://form.jotform.com/OktaTraining/Premium (upper case P)
only some of the check boxes appear.
I have tried clearing both the cache and cookies but it did not help much. In both your links can you place both URL link with the following URL
https://form.jotform.com/OktaTraining/premium
This should resolve the issue.
