We are creating a form. The link is below. Upon hitting submit, I am receiving a "This form is currently unavailable" response instead of the thank you page, and the results are not being recorded.
This has been happening since last Friday. I had assumed that maybe it was a JotForm server issue and would give it the weekend in case it was. However, it is still happening today, Tuesday, June 28.
Please let me know what I may do to fix this issue.Page URL:

Form that you provided is showing "This form is currently unavailable" message, because you have set Expiration Date inside of your form preferences. Date 2015 October 28 that you have set is in the past and because of that your form is showing mentioned message.
You need to disable expire on date option or to set date that is in the future.
