To my understanding you are having an issue with receiving your form notification and autoresponder to your email address.
I checked your form and saw you have the right setup but I see you are using your own email address as the sender and that is known to cause this type of problem and it is recommended to use the default firstname.lastname@example.org or email@example.com.
I would suggest trying this with one of those then do another test submission by filling in your form so that you can test each more accurately on your form to see if it arrives or not.
