Edit text in heading and send test form email

If you look at my form, GBwineorder, note the heading with the logo.  I can't figure out how to get that address into single lines, and so the zip code is in the first line with the street address.  How can I make each line of the address separate?

 

 I want to complete the form and hit submit and see what I get on the form.  How can I do that?

 

When reviewing your form I noticed that the header, which contains the address is already in a single line.
