How do i open a survey after i have closed it?

Once you will login to your account under the "My Forms" tab, you will get all the Forms created by you, please check the screenshot:

I think recently you have interacted with this Form: https://www.jotform.com/61405473141952

So please check it, and if you mean some thing else, please revert back.
