I cant find my form and i can't log in

https://form.jotform.com/61118621594152 is my form, it won't let me login .  Its saying username,password is not valid. HELP.  I'm not getting my surveys

I'm not sure if you had an account since the form you have shared us belongs to a guest account, maybe this is the reason why you cannot log in.

A guest account will be available as long as you do not delete your browser storage, I would suggest you to create an account first and then you can request us on this thread to move the form to your created account.
