When I try to log in it says my account has been suspended for phishing. I have no fields trying to collect credit card info or email info. The account email is trevor@thomasduke.com. This happened earlier as well but it eventually let me log in.

I have checked that form and it seems to be good, you're not currently asking for CC, login nor other info that is not allowed at JotForm.
I have activated your account and your forms should be loading fine now, although you have your forms without any inconvenient, I would like to share you the page to our terms, so you avoid any suspension due to phishing: http://www.jotform.com/terms/

Hi,
We have just sent out 500 invitations, and our Jotform account is now blocked for "Phishing"
Could you please help opening it up again?

I checked your forms and any problem with to violet our Terms. I activated your account again.
There is a problem why your translation. Maybe, our phishing detector took your fields like password, credit card etc.
