Trouble integrating Salesforce with form.

I've been following the instructions here https://www.jotform.com/help/271-Flexible-Data-Integration-With-Salesforce In the section "match your fields" when I click on "Please select" to choose or add a value for the field, no options to change it are presented. I've authenticated and filled everything else out. I've hit a wall, and want to get past this part. I appreciate your help.

When reviewing your TEST form, you did not add any fields except the header and submit button. You are require to add at least one field so it can be mapped in your SalesForce integration.

Once you have added a field this will be able to be selected in you SalesForce Wizard.
