To access form designer, is there a cost? When I used it last night, there was no save button or ways to transfer.

The Form Designer is available for all the plans including the Free plan. We don't have any reported issue about the problem you're describing.
Are you still experiencing the same problem? Please try to use another browser to check if the tool will work properly or not.

Yes, it does work now. I had to "save" first before I was able to up load.
But, another question came up.... Is there a way to arrange or align the "send" button (box) with the email box? I'm actually trying to create an email list for people viewing my web site.
